What
to Bring to Your Appointment
Becoming a Patient
Prior to making an initial
appointment, a nurse from the Loyola
CAN-HELP Cancer Information Service (specialized
in helping people with your particular illness) will
ask you a series of questions over the telephone. These
questions are designed to assess your condition and
help determine the appropriate approach during your
initial visit.
When patients and families
come to the Cardinal Bernardin Cancer Center for the
first time, we ask that you bring the following items:
- Written medical records,
including lab and treatment information to date
- Pathology slides and
reports
- Chest X-rays and reports
- CAT scans, MRI scans
and reports
- Operative reports, if
appropriate
It's always a good idea
to discuss what to bring with the person who makes the
appointment for you. You will be asked to verify insurance
coverage and instructed to secure a referral from your
primary care physician, if appropriate.
